APPLICATION FOR MEMBERSHIP
ARRT BOARD ELIGIBLE IOWA RADIOGRAPHY STUDENT

We offer you a gift from the Iowa Society of Radiologic Technologists (ISRT). This is for being ARRT (Radiography) Board eligible via a Recognized Iowa Radiography Program. We offer you a 1-year membership to the ISRT, free of charge.

The membership year is either July 1 – June 30, or one year from the end of your current membership.

It is our hope, that this will help you in your 1st year out of school and that you would renew this ISRT membership annually thereafter.

    I have a current membership with the A.S.R.T. (American Society of Radiologic Technologists)

    With your form submission, you hereby make application for membership in the Iowa Society of Radiologic Technologists and agree to support the Constitution and Bylaws of the Society and conform to the code of ethics adopted by it.
    The deadline for submission is June 1. You will receive your electronic membership card on or about July 1.
